  I am aware that any violation or infringement of the form owner's intellectual property rights may prompt the form owner to initiate legal proceedings against me to safeguard their proprietary interests. Such legal actions may lead to the imposition of appropriate remedies and compensatory measures, as prescribed by the applicable laws, to redress any losses incurred as a result of my unauthorised use.
  
  This acknowledgment and commitment epitomise my understanding of the critical importance of respecting the intellectual property rights vested in the Google Form and its contents, as well as the legal repercussions that may ensue if I breach this obligation.
   

• Disclaimers and Limitations of Liability: I acknowledge and fully understand that the Google Form, including all its contents and information, is provided on an "as is" basis, without any express or implied warranties. I hereby affirm that the form owner makes no representations or warranties regarding the completeness, accuracy, reliability, suitability, or availability of the Google Form or any information, products, services, or related graphics contained therein.
  
  I explicitly release the form owner from any and all liability for any direct, indirect, incidental, consequential, or punitive damages that may arise from my use of the Google Form or my reliance on any information provided within. I further recognize that the form owner shall not be held liable for any losses or damages incurred due to the unavailability, interruption, or termination of the Google Form or any associated services.
  
  This acknowledgment signifies my understanding of the inherent risks associated with using the Google Form and my acceptance of the form owner's disclaimers and limitations of liability. I hereby waive any potential claims or legal actions against the form owner for any damages resulting from my interaction with the Google Form, its contents, or any related materials.
  
  By accepting these disclaimers and limitations, I affirm my intention to act responsibly and prudently while using the Google Form, taking into account the absence of warranties and the form owner's exclusion of liability for any potential losses or damages.
   

• Indemnification: I hereby agree to indemnify, defend, and hold harmless the form owner, its affiliates, and their respective officers, directors, employees, agents, licensors, and suppliers (collectively referred to as "Indemnified Parties") from and against any and all claims, actions, suits, or proceedings, including associated losses, liabilities, damages, costs, and expenses, including reasonable legal fees, arising out of or in connection with my use of the Google Form, any violation of these terms and conditions, or any breach of applicable laws or regulations.

  This indemnification provision shall extend to cover all losses, damages, liabilities, and expenses incurred by the Indemnified Parties as a direct or indirect consequence of my actions or inactions while using the Google Form. I agree that this indemnification obligation shall survive the termination of my use of the Google Form and remain in effect until all claims and liabilities covered herein are fully resolved and discharged.
  
  This undertaking serves as a legally binding commitment on my part to protect and safeguard the form owner and the Indemnified Parties from any adverse consequences that may arise from my use of the Google Form or any failure to comply with these terms and conditions or applicable laws.
   

• DND Call Consent: By providing my consent, I acknowledge and affirmatively allow the form owner or their representatives to contact me by phone, notwithstanding the fact that my number may be registered on a national or state Do Not Call Registry or any other similar registries.
   

• I comprehend and agree that these phone calls may serve various purposes, including providing informational updates or engaging in marketing communications. However, I retain the unrestricted right to decline to receive such calls at any time, and I may exercise this right by contacting the form owner directly or adhering to the opt-out instructions furnished during the call.
   

• This consent signifies my voluntary agreement to permit phone communications from the form owner or their representatives, despite any regulatory restrictions. I acknowledge that this consent remains valid and in effect unless revoked by me in writing or through other means by the form owner.
   

• This provision serves as a legally binding authorisation for the form owner to contact me by phone in accordance with the terms outlined above, while also acknowledging my entitlement to opt out of such communications at my discretion.
   

• Privacy Policy: I hereby acknowledge and explicitly recognise that the form owner collects Personal Information from me for the purpose of providing its services. I understand that my Personal Information may include but is not limited to, my name, contact details, and any other data provided by me through this Google Form.
   

• I affirm that the form owner shall not disclose, share, or transfer my Personal Information to any third parties, except in circumstances where required by law or upon obtaining my express consent. My Personal Information shall be treated with utmost confidentiality and safeguarded against unauthorized access or disclosure, in strict compliance with the applicable data protection laws and regulations.
   

• I acknowledge that the form owner shall implement reasonable and appropriate security measures to protect my Personal Information from unauthorized access, alteration, or destruction. These measures may include encryption, access controls, and regular security audits.
   

• This acknowledgment signifies my informed consent to the collection and processing of my Personal Information by the form owner for the purposes stated in this Google Form. I retain the right to withdraw my consent at any time by contacting the form owner in writing.
   

• By providing my Personal Information and submitting this Google Form, I agree to be bound by the terms of the form owner's Privacy Policy and authorise the form owner to process my Personal Information as described herein.
   

• Modification: I hereby acknowledge and explicitly recognize that the form owner reserves the unilateral right to modify, amend, or alter these terms and conditions at any time, without the obligation to provide prior notice to me. Such modifications may be made for any reason deemed necessary by the form owner, including but not limited to, changes in legal requirements, operational enhancements, or service updates.

• You acknowledge and understand that the form owner may use the contact information you have provided to communicate with you through various means, such as email, phone calls, text messages, or other forms of communication. Furthermore, you affirm that you have the authority to provide the contact information and consent to be contacted using those details.

Withdrawal of Consent: If you disagree with any of the terms and conditions presented in this Google Form or if you wish to withdraw your consent for any reason, you retain the option to refrain from submitting the form. Your decision to not submit the form signifies that you do not provide your consent to the terms and conditions, and no data or information will be collected from you.
